At Strathalbyn Physiotherapy, we support older adults who are receiving, transitioning to or applying for Support at Home services. Our physiotherapy is designed to help people maintain mobility, improve strength and balance, reduce falls risk, recover after hospital admission or surgery, and keep doing the daily activities that matter.

What is Support at Home?
The Australian Government's Support at Home program replaced Home Care Packages and the Short-Term Restorative Care Programme on 1 November 2025. It is designed to help older people remain living at home for longer by improving access to care, services, equipment and home modifications.
Under Support at Home, older people can receive coordinated support based on assessed needs. This can include clinical supports such as physiotherapy, nursing and other allied health services, along with independence and everyday living supports.
If you are unsure how your funding works, your care manager or provider can help confirm what physiotherapy services are available in your support plan. The official Australian Government information is available at My Aged Care .
How physiotherapy can help
Physiotherapy is often one of the most useful supports for older adults who want to remain independent, safe and active. Our focus is not just treatment of pain. It is helping people move better, reduce risk, build capacity and stay engaged in daily life.
Physiotherapy may assist with walking, balance, strength, transfers, falls prevention, confidence after a fall, recovery after hospital admission, arthritis management, post-operative rehabilitation, neurological conditions, mobility aid prescription and exercise programs matched to your goals.
Our reablement-focused approach
Support at Home places strong emphasis on wellness, independence and reablement. That means therapy should help a person regain, maintain or improve the skills they need for everyday life, rather than only providing passive care.
At Strathalbyn Physiotherapy, we build treatment plans around practical goals such as getting out of a chair safely, walking to the letterbox, climbing steps, returning to gardening, improving shower transfers, reducing fear of falling, or rebuilding strength after illness.

Mobility assessment and walking confidence
Assessment of gait, walking tolerance, mobility limitations, confidence, falls history and practical barriers to moving safely at home and in the community.
Falls risk assessment and prevention
Identification of personal risk factors such as weakness, balance impairment, dizziness-related caution, pain, reduced reaction time, poor confidence, unsafe transfers or unsuitable mobility aids.
Strength and balance programs
Individual exercise programs to improve lower limb strength, posture, control, balance reactions, endurance and confidence with movement.
Post-hospital and post-operative rehabilitation
Support after hospital admission, joint replacement, fracture, general surgery, deconditioning or a period of reduced activity.
Mobility aid advice and review
Advice regarding walking sticks, frames or four-wheeled walkers where clinically appropriate, including fit, safe use and functional training.
Pain and arthritis management
Education, hands-on treatment where appropriate, exercise and pacing strategies to help people manage pain while maintaining function.
Neurological physiotherapy
Support for people living with stroke, Parkinson's disease, multiple sclerosis, brain injury, spinal cord injury or other neurological conditions through our NeuroTherapy Centre.
Restorative care and short-term goal blocks
A defined therapy block focused on specific functional goals, especially after illness, falls, hospital admission or a clear decline in independence.
How we deliver Support at Home physiotherapy
Support at Home physiotherapy at Strath Physio is for people living in their own home. We do not provide visits to residential aged care facilities under Support at Home packages.
In-clinic appointments
Most appointments take place at our purpose-built clinic at 44 High Street, Strathalbyn. The clinic includes dedicated exercise space, balance and strength equipment and our NeuroTherapy Centre, making it well suited to mobility, strength and reablement work.
Home visits, case by case
Where it is clinically appropriate and practical, we offer home visits for Support at Home clients. Home visits are arranged on an individual basis depending on your location, care plan and clinical priority. Our admin team will discuss the arrangement that suits your goals before any appointment is confirmed.
Working with your aged care provider
Strathalbyn Physiotherapy works as the physiotherapy partner for a number of registered aged care providers across our area. You can see the providers we work with on our community partners page.
To remain a preferred partner of these providers, our clinicians and clinic undergo annual compliance and police checks, including the requirements set by the Department of Social Services and Aged Care regulators. This gives clients, families and care managers confidence in how we work.
Our admin team will triage your enquiry and match you with the physiotherapist best suited to the complexity of the case. We can liaise directly with your care manager or provider on goals, appointments, reporting and invoicing.

Yes, physiotherapy may be included where it is part of your assessed needs and support plan. Support at Home includes clinical supports such as allied health services. Your care manager or provider can confirm whether physiotherapy is available within your current arrangements.
Support at Home replaced the Home Care Packages Program and Short-Term Restorative Care Programme from 1 November 2025. Some people transitioned from HCP into Support at Home, and some people have grandfathering arrangements that affect contributions.
Physiotherapy can help with strength, walking, balance, falls prevention, post-hospital recovery, post-operative rehabilitation, arthritis management, pain, mobility aid use, neurological rehabilitation and confidence with daily activities.
A GP referral is not always required for physiotherapy, but Support at Home clients should confirm their funding pathway with their care manager or provider. Bringing relevant medical and care plan documents helps us plan treatment.
Yes. We provide both in-clinic appointments at 44 High Street, Strathalbyn and home visits on a case-by-case basis, depending on your location, care plan and clinical priority. Yes. Falls prevention commonly includes strength and balance training, mobility aid review, transfer practice, gait retraining, education and practical strategies to improve confidence and safety.
Yes. With the client's consent, we can communicate with carers, family members, care managers and other health professionals so therapy goals are clear and practical.
